This site was developed to provide a range of instruction and resources to improve the communications capacity of the grantees of the John A. Hartford Foundation.

Guided Care.org
This organizational site is part of a larger diffusion effort aimed at spreading the use of a new model of team-based care for older adults with chronic conditions and complex health needs.

Infoaging.org
Launched in 1999, this 700+-page site is the Web’s most comprehensive source for the latest information about research about the biology of aging, age-related diseases and conditions, and healthy aging.
